Early On® Process for Families
Transition
Transition planning and support should be an integral part of the individualized family service planning process from the very beginning. To view the Early On Transitions Guidebook click here. Transition steps are included in an IFSP to ensure that each child and family is prepared to move from one set of services to another and from one setting to another. Those steps include:

discussions with, and training of, parents regarding future placements and other matters related to the child’s transition |
• |
procedures to prepare the child for changes in service delivery, including steps to help the child adjust to, and function in a new setting; and |
• |
with parental consent, the transmission of information about the child to the local educational agency, to ensure continuity of services, including evaluation and assessment information required, and copies of IFSPs that have been developed and implemented. |
A face to face meeting (conference) with the family and other required providers takes place not less than 90 days before the child’s third birthday or eligibility for preschool services under Part B of IDEA. Effective planning for a child’s transition at age three can help families learn strategies to plan and manage the many transitions that will occur throughout their child’s educational experience.
Guidelines for Transition from Early On
This document is intended to assist service providers in partnering with families in the transition process for Part C eligible children to Part B services and/or other community based services, many of which may not be affected by the child’s third birthday.
